Look at your state specifically to see income requirements. 


The Children's Health Insurance Program (CHIP) provides health coverage to nearly eight million children and families with incomes too high to qualify for Medicaid, but can't afford private coverage.

In some states, CHIP covers parents and pregnant women. Each state offers CHIP coverage, and works closely with its state Medicaid program. Please note: These programs may be called by different names in your state.

Some benefits covered through CHIP, include:

  • Routine check-ups

  • Immunizations

  • Doctor visits

  • Prescriptions

  • Dental and vision care

  • Inpatient and outpatient hospital care

  • Laboratory and X-ray services

  • Emergency services

Program Requirements

CHIP serves uninsured children up to age 19 and pregnant women age 19 and over with incomes too high to qualify them for Medicaid. In general for this benefit program, the child must be a U.S. national, citizen, legal alien or permanent resident, and have low income. The child must also be a resident of Florida.

In order to qualify, you must have an annual household income (before taxes) that is below the following amounts:

*For households with more than eight people, add $8,360 per additional person. Always check with the appropriate managing agency to ensure the most accurate guidelines.

Florida Medicaid is the state and Federal partnership that provides health coverage for selected categories of people in Florida with low incomes. Its purpose is to improve the health of people who might otherwise go without medical care for themselves and their children.

Program Requirements

In order to qualify for this benefit program, you must be a resident of the state of Florida, a U.S. national, citizen, permanent resident, or legal alien, in need of health care/insurance assistance, whose financial situation would be characterized as low income or very low income. You must also be either pregnant, a parent or relative caretaker of a dependent child(ren) under age 19, blind, have a disability or a family member in your household with a disability, or be 65 years of age or older.

In order to qualify, you must have an annual household income (before taxes) that is below the following amounts:

Children's Medical Services Managed Care Plan is designed for those individuals who are Medicaid eligible and have special health care needs.

How does a child qualify?    

  • Must meet Medicaid financial requirements

  • Must be under age 21

  • Must meet CMS clinical screening requirements or have a physician attest to child's qualifying medical conditions

How do I request a clinical eligibility screening or rescreening?

To request a CMS Plan clinical eligibility screening for the CMS Plan for Medicaid, KidCare or Safety Net*, please contact your local CMS Plan area office. You may request a rescreening at any time. If you have already participated in a screening and would like to request a rescreening, please call your local CMS Plan area office or 1-855-901-5390.
